[Partnerships] survey questions for out-of-school youth & agencies

Hi -

I am forwarding this request on from a program in the
West to the list.....

We are hoping to do a short survey of out-of-school
youth.  We came up with some questions last fall, but
I am not sure if they are the best questions.  If any
groups have survey questions for out-of-school youth,
we would find them very helpful.  Also I think that we
hope to do a short survey (5 questions) with social
service agences, etc. I found one question about
referrals that we could use.  But getting ideas of
other questions that we could include in a short
survey would be very useful.  Thanks to all for your
help.
