Hi Margery,

Thanks for letting others know about the survey.  Actually I would
prefer it if they not format the survey in the text of the email.
Instead, they should request the email version of it directly from me
or download the hardcopy version from the web address I included..
That way we reduce the possibility of the survey inadvertently being
changed in some way, which would make the responses invalid.
