Thanks for all the information and the Consent Form. 
Although the pieces seem to coming together pretty well
it's nice to see them in writing so we can all nod our heads 
or start posting questions...

BTW ... I am quite proud to say that  I was Dori's Mc.'s
first practice assessment yesterday and it went pretty
well ... I think I came out as a mid-high SPL level.
