[Bestplus] Training form

Hello all,

Attached are two forms that will be used as part of your BEST Plus
trainings, as explained in the Expectations and Guidelines.

- BEST Plus Proficiency Development Form:  You will give one of these
forms to every participant who completes the full six hours of training.
 They will then use this form to document their required 10 practice
tests (directions included).

- BEST Plus Training Evaluation Form:  You will also give one of these
forms to every participant who completes the full six hours of training.
 You will then collect these and send them back with other documents to
your SABES RSC.

While your RSC will normally provide photocopies of these forms as part
of your training materials, please print out hardcopies to use as
back-ups.
