Andrea, actually my experiential observation and the point of my inquiry is
that many of the methods in Tom's review ARE used, and have been at least
since I started my career in 1978.  I asked about research into the
question because I wonder if the problem is that the methods aren't used
enough, or widely enough, or just don't work with some, or many, people. 
My hunch is the latter may be true.  Debbie
